Dear Sir/Madam, I would like your opinion. I have impacted lower wisdom teeth. I am 39 years old and supposedly wisdom teeth are more difficult to remove with age. I have had problems for about 1.5 years. I had pain and was sent from one doctor to another; most recently I was told the teeth are responsible. Because Sjögren's syndrome was also suspected, I have been taking Medrol 4–8 mg daily for a year, although I had also taken it occasionally before during asthma exacerbations. I should now gradually discontinue it, but when I reduce the dose I have more problems—wheezing, painful and difficult swallowing... Because I currently have painful lymph nodes on the right side under the jaw and difficulty swallowing, I was prescribed Hiconcil 500 every 8 hours. I have already taken a box and a half, but the condition is hardly any better. I have pain in the jaw towards the ear, feel pressure, etc. The surgeon is currently away and I do not know what to do. The teeth are right next to the nerves; the report says they are in a very unfavourable position, tilted towards the tongue, so the jaw is very thin on the lingual side. At the University Medical Centre Maribor ENT-Maxillofacial Surgery department I was offered removal of both at once under general anaesthesia, while my oral surgeon would remove them one at a time. I therefore do not know which option to choose and what you recommend. Also, how does Medrol affect the procedure, am I taking high doses, and what are the consequences of taking it before and after surgery? I should also mention that with the first oral surgeon I had already been scheduled for surgery last year. I brought him the panoramic X-ray and he scheduled me for November and December; I waited approximately several months, and then one week before the operation he cancelled it, saying he would not do it. I feel he could have told me that at the examination instead of making me wait almost a year, so now I have to look for another surgeon and wait again. Please understand that I have written at length. I am frightened, psychologically I am completely exhausted and I have lost 12 kg because of the pain and the way I have had to eat—liquid and puréed food. I hope you can send me an opinion and tell me what kind of procedure awaits me. Kind regards, Simona from Maribor

Hello,
about 1 month ago I went to a dentist who, over a period of 14 days, replaced my amalgam fillings with white fillings and also repaired several teeth. Altogether he treated 7 teeth. Since then, for a month now, my teeth have been hurting: sometimes the upper jaw, then it moves to the lower jaw, etc. Some of the fillings were indeed very large, but still this pain is unbearable and for a month I have been taking analgesics every day. I read that this can be a reaction after receiving a white filling, but if that is the case I would rather have kept the amalgam fillings forever, because my teeth are now also very sensitive to heat or cold. What do you think—is this normal?
Thank you.
Kind regards,
Valerija

Dear Sir/Madam!
If we are talking about tooth sensitivity to changes in temperature or osmotic pressure, the condition is reversible, meaning it can return to normal, but this takes time (possibly six months or more). The discomfort can be reduced by using resins to impregnate the cervical areas of the teeth. If the pain becomes spontaneous or becomes stronger on biting or tapping the tooth, the condition is irreversible and requires additional root canal treatment. The problems you describe are a consequence of treatment on the tooth, which of course depends on the depth of the carious processes or existing fillings. Every procedure on a tooth represents considerable irritation to it, which may lead to problems, but what is necessary is necessary.

I assume you are missing three incisors. The missing teeth can be replaced in two ways:
-making a bridge, which requires using at least two adjacent teeth as abutments. Altogether at least five crowns therefore have to be made. The bridge is made in 10 days, or three visits. Price:
-EUR 210 (metal-ceramic crown) X 5 = EUR 1050.
-placement of two implants and fabrication of a three-unit bridge. Price:
-EUR 750 (implant) X 2 = EUR 1500
-EUR 420 (crown on implant) X 2 = EUR 840
-EUR 210 (crown or pontic) X 1 = EUR 210.

Dear Sir/Madam,
I have developed severe pain (lasting at least 4 days) in the area of a “porcelain crown” (lower tooth 5) that I received approximately 10 years ago.
It hurts around the tooth (the gum, which has receded considerably) and at the top if I tap or bite. The emergency dentist inserted some strip—for a few hours it seemed better, but now it hurts badly again (I have already had to take several Analgin tablets). I have no swelling on the outside of my face, and the gum is not swollen either.
Please let me know when you could see me tomorrow, at least enough to perform a basic pain-relieving procedure; we could arrange the rest afterwards.
Respectfully,
Robert

Is it possible to place implants in the upper jaw if a 3D panoramic scan showed that the bone is only 3 mm thick? What is the procedure and how much does it cost in the case of 4 implants, bone augmentation and a removable denture? Thank you in advance for your answer.

Dear Sir/Madam! In the case you describe, I assume that you need a sinus lift. In the worst-case scenario, the price is: -EUR 1000 (implant with sinus lift) X 4 = EUR 4000 -EUR 900 (denture with cast base). If you do not need a sinus lift, the price is: -EUR 750 (implant) X 4 = E

Hello,
I have a 4-year-old daughter. She had pain in an upper primary first molar; when the filling was removed, there was pus.
A week ago she received a new filling, and today I noticed that the gum is inflamed again and there is pus again.
What is the next step: should removal of the dental nerve be requested? Because if she gets another filling (after treatment of the gum), I think we will have the same problems.
I would also like to know how much this medical procedure costs at your clinic.
I kindly ask for your answer, thank you!

Dear Sir/Madam!
The primary tooth is certainly non-vital. Conventional root canal treatment and root canal filling are not appropriate here. In practice, such a tooth is most often left open until it falls out, or it is filled with filling materials that are mildly bactericidal and then restored. Recurrent problems (swelling, pain) are not completely excluded by this.
