Dear Sir/Madam!
During extraction of upper molars or premolars, the condition you describe often occurs. It means that there is an opening between the oral cavity and the paranasal sinus at the site of the extracted tooth. Most such communications close spontaneously, but you need to be careful about increasing pressure in the paranasal sinuses (straining, diving...). Smoking certainly has a negative effect on any wound healing.

Hello!
I kindly ask for an answer regarding a solution to the following problem:
Both upper central incisors and the upper left lateral incisor are non-vital. The upper right central incisor started hurting badly. The panoramic X-ray shows cysts on both central incisors, although the left one does not hurt yet. My dentist gave me 3 tablets of Sumamed, but since there was no improvement, she gave me another 3 tablets of Sumamed. She also incised the gum above the tooth, expressed some pus and inserted a strip. The pain still persists and I control it with Naklofen. I was also prescribed another antibiotic, Klindamycin 300 mg, which I am still taking. Because I have prosthetic work on the lateral and central incisors that also connects other teeth, she mentioned an apicoectomy, but only on the painful central incisor. I would like it done on both because both have cysts. I also have other problems: I have already been hospitalized twice because of dizziness, vomiting and headaches that appear suddenly, and the cause has not been fully determined. Investigations are still ongoing. I also have a meningioma. Because of all the current problems and possibly further ones, I would like to solve at least the dental problem as soon as possible, so I am asking how long the waiting time is at your practice, whether in view of the other problems mentioned the procedure can be carried out soon, and of course what the price is.
Thank you sincerely for your answer and all the best!

For some time now I have been having problems, namely an indistinct pain in the front teeth; I constantly feel the teeth, and swelling and gum inflammation have also started to occur. Because of the pain, my dentist has already devitalized both upper central incisors, but nothing is better. Since I can see that my dentist has no idea what is causing the pain, I would like advice on how to approach the problem.

I certainly believe that your dentist has done everything that was possible under the circumstances. Based on the problem described, I think there are two possibilities: repeat root canal treatment of the teeth in question or, if that is not possible, an apicoectomy. To decide, I would need to see the X-ray.

I am not an orthodontist. In any case, the best solution is the one proposed by the specialist: arranging the position of the teeth and, of course, subsequently replacing the lateral incisors with dental implants. If you reject implants, the lateral incisors can be replaced with a conventional bridge, but this usually requires at least grinding both canines.

Dear Sir/Madam!
The gap can be closed partially or completely with composite material (white filling). The procedure is reversible and non-radical because it does not require grinding the tooth surface. The result is good, but composites can discolor over time or come off under excessive loading. Price:
-65 Eur (build-up per tooth) X 2 = 130 Eur.
The price includes all materials and procedures from the beginning to the final result.

I do not want braces. I would like to know what the procedure would be (grinding, crowns...?) to straighten the tooth so it is nicely in line, or at least so that it does not ''stick out'' too much. How much would such a correction cost me?

If you have ruled out straightening the tooth with braces, you have several options:
-reshaping the tooth with composite material (white filling), which is a reversible and non-radical procedure. Price: 65 Eur
-reshaping by making an aesthetic veneer, which in most cases requires altering the intact enamel. Price: 350 Eur.
-reshaping with a crown is a radical procedure that requires grinding the dental enamel on all tooth surfaces. Price: 350 Eur.
The choice of procedure depends on the size of the defect or the degree of reshaping we want to achieve. The best solution is moving the tooth with braces.
