We recently had a medical examination at school and the dentist gave me a note saying that I have caries and should visit a dentist as soon as possible. I had already noticed a black hole myself, which seemed quite deep. The dentist told me that it looks only superficial, but is probably deeper. I would like to know whether I will get a filling, because I really do not want one.

Dear Sir/Madam!
If we wish to avoid orthodontic therapy, fixed prosthodontic rehabilitation is required, which means reshaping the dental crowns to achieve the desired aesthetic and functional effect. Compared with orthodontic therapy, the procedure is radical, because the teeth must be partially ground down, which is irreversible. Price of the procedure: -210 Eur (metal-ceramic crown) X the number of crowns you would like to reshape, or -350 Eur (all-ceramic crown; Zircon) X number of crowns. The price includes all materials and procedures from the beginning to the final result.

For some time I have had problems with the upper left second premolar. When I pointed this out to my dentist, he X-rayed the tooth and said that I had exposed enamel, but otherwise there was nothing wrong with the tooth. Then, because of bleeding gums, I started seeing a periodontist, who performed the hygiene phase in May 2010 and then flap surgery in all four quadrants in August and September 2011. In May 2012 they cleaned dental calculus at the periodontist. After that this tooth became very sensitive and painful. I visited my dentist, who opened the tooth and replaced the filling, and said everything was fine with the tooth. After a few days severe pain developed. I visited the emergency dentist, who opened the tooth and cleaned the canals (25-trepanation; B-19, P-17; Calosept, Cavit). This was repeated several times until August 2012, when the dentist believed the tooth had healed and filled the canals and closed the tooth. A few days later severe pain started. I was given Amoksiklav 1000. It was fine for a while, then the pain returned. In November 2012 the filling was removed. In December 2012 the canals were cleaned again, medication was inserted and the tooth was temporarily closed. After a few days I developed swelling and the tooth hurt. They again removed the temporary closure and cleaned the canals — very painful. On 22 December 2012, pain again, swelling and a pus-filled bump above the tooth. Again Amoksiklav 1000, two boxes. Please give me your advice. Kind regards.

Dear Sir/Madam!
For the diagnosis and prognosis of the tooth in question, an X-ray must first be taken. If it is possible to fill the root canals in a radiographically indisputable manner and the tooth nevertheless continues to cause symptoms (pain, swelling...), we usually resort to apicoectomy or root-end surgery. The reliability of this surgical procedure is relatively low (approx. 70%), but it is the first treatment of choice if the tooth cannot be adequately treated conservatively (root-canal treatment). If the apicoectomy is also unsuccessful, the tooth must be extracted.

Dear Sir/Madam!
In the situation described, it is best to visit a dentist. They will try to resolve the inflammation of the periodontal tissues with medication. If conservative therapy is unsuccessful, it is possible to remove part of the soft tissue covering the crown of the wisdom tooth (decapsulation), which makes the area easier to clean and essentially prevents the problems from continuing. Extraction of the wisdom tooth is used as a last resort.

Since my next appointment with the dentist is not until mid-January, I would like to know whether this is normal and whether I simply need to put up with it for a while until the condition improves.

Dear Sir/Madam!
The prognosis and diagnosis of periodontal disease are established by examining a panoramic X-ray and performing a clinical examination that includes measuring the depth of periodontal pockets. These diagnostic procedures are essential for selecting the treatment method. We usually begin treatment with calculus removal and instruction in proper oral hygiene. This is followed by scaling and root planing and gingival curettage. If the pockets are deeper than six millimetres, flap surgery and GTR (guided tissue regeneration) are performed.

Hello. I would like to ask for your opinion. I had my upper left central incisor left open for two years, and after two years I finally found the courage, because I have an abnormal fear of dentists, and went to a dentist. He started cleaning the canals; he also had an X-ray of the tooth and decided that we would try to save it so that I could keep my own tooth. When he started cleaning it hurt a little, but when he reached the root tip the pain was terrible... somehow I endured it. He put in medication and, just in case, prescribed antibiotics in the event of swelling. And indeed, from Thursday to Friday a swelling developed above the tooth which was very painful to touch but otherwise not. From Friday to Saturday the swelling spread to the entire left half of my face, my eye closed, etc. I went to the emergency dentist, who reopened the tooth and made an incision above it so the pus could drain. I have now been going for a week to have the strips changed and I am taking antibiotics. The diagnosis is a dental abscess, and a truly terrible abscess... the swelling is gone but there is still a hard lump above the root that hurts unbearably even when I touch it just slightly. And the tooth is completely black or very dark, while my right central incisor is nice and white. Now my dentist has decided, or told me, that the solution is extraction of the tooth — to have it extracted and later get a replacement or implant. I would like to know whether you would perhaps make the same decision or advise extraction given my condition. I would be very grateful for an answer. Thank you very much. Kind regards.

Given that the tooth was left open for two years, its prognosis is very poor. It is always worth trying, but in view of the clinical picture you describe, I believe it would be best to remove the tooth. A final decision can only be made after examining the X-ray, which is also essential for root-canal treatment itself.

Kind regards. This week I became very ill with the flu and I can see that it has affected my teeth... It started with swelling of the gum between the upper right central and lateral incisors. A day later it spread to the right canine and from there all the way to the left canine in the upper jaw, including the central and lateral incisors. The next day, when I woke up and wanted to brush my teeth, my upper gum started burning badly. When I looked more closely in the mirror, I saw that the edge of the gum had lifted away from the teeth and started bleeding. I would like to know what procedures this requires and what the waiting time is, given that everything is spreading quickly, and if you could also explain the reasons why this occurs. Thank you very much.

Dear Sir/Madam!
The systemic processes in the body that you describe can significantly alter the immune response in the oral cavity as well, so certain disease conditions may coincide in time. Judging from what you have written, this is acute gingivitis, i.e. inflammation of the gums, which is certainly not caused by the flu alone, but has probably been smouldering for years as chronic inflammation (periodontal disease) and, because of its advanced form, has flared up due to the circumstances described above. First, the stage of periodontal disease must be determined (panoramic X-ray), and the underlying disease then treated accordingly (oral-hygiene instruction, removal of calculus, root scaling and planing, surgical procedures...). Given the clinical picture described, aphthae could perhaps also be causing the problem.
