Hello
I am 24 years old and I have a problem with my teeth. My permanent teeth erupted sometime between the ages of 8 and 11; at 13 I got two fillings, in the upper right first molar and the lower left first molar. About a year ago, however, I began having problems with the upper first and second premolars. In less than a year both first premolars and both second premolars in the upper jaw completely broke down; of course the teeth can no longer be saved and have to be extracted.
The other teeth are fine. I would like to know what could be the reason that all of these upper premolars suddenly broke down like this.
I have not found a similar problem described anywhere.
Thank you for your answer
Have a nice day

Dear Sir/Madam!
The situation you describe is most often caused by carious changes in the affected teeth that were not treated in time. Most commonly this is decay on the contact surfaces, which is more difficult to detect, so the tooth ultimately decays completely and collapses in on itself. In rare cases the cause is a congenital disorder in the composition of the hard dental tissues. In such forms of disease, carious processes also spread much faster and reach the kind of problem you describe much sooner. For a more precise answer, you would need to come to our practice.

Hello!
I have two questions concerning wisdom tooth extraction. I would like to know whether we have to be completely healthy for the procedure, or whether it is still performed if the patient has a cold or a mild cold.
My second question is how long, on average, wisdom tooth extraction takes at your practice. I have already had one wisdom tooth (lower right) removed at the Dental Clinic in Ljubljana, where the procedure lasted a full two hours, and you can imagine that I did not have a good experience. Is it possible that some people simply have teeth that are more difficult to remove, or did the fact that the surgeon was still a resident with little experience contribute to my two-hour ordeal?
I am considering booking the removal of the remaining three wisdom teeth with you.
Thank you for your answers and kind regards!

Dear Sir/Madam,
My question concerns a dental bridge. Seven months ago my dentist fitted a bridge. The upper left lateral incisor was missing, so the upper left canine (which was healthy, of course) was prepared and now supports the missing unit. The bridge is metal-ceramic. My problem is the following: even after seven months, the canine to which the bridge is attached still hurts when I bite, when it comes into contact with cold or very hot substances, and sometimes it even hurts when I breathe cold air. Because of these problems I have already been to my dentist three times, and the answer is that this is normal because the tooth underneath is vital and because the metal beneath the ceramic is a strong conductor of heat, cold and the pressures created when chewing food; he also says that I will have to get used to the foreign body, but he does not tell me how long this adaptation takes. I would not be writing to you if I were not already very worried that something had gone wrong with my bridge, and if I had not already been to my dentist three times because of this. With all due respect, I ask you to tell me what I should do and how to proceed. I should add that all my other teeth are fine and that only this lateral incisor, now replaced by the bridge, had caused me problems for 15 years, which is why my dentist had to extract it last year because I had developed a hidden granuloma.
Thank you for your answer.

Dear Sir/Madam!
The condition you describe is very common. The canine was considerably affected during the preparation procedure. Removing enamel with burs is a radical procedure that can leave temporary or permanent consequences on the tooth. As long as the tooth is hypersensitive to temperature stimuli, we speak of a pre-inflammatory condition of the dental pulp. The condition may improve on its own because a vital tooth forms tertiary dentin inside the pulp chamber, which protects the pulp from external stimuli. The time required for sufficient self-protection can be up to one year. If the tooth becomes sensitive to biting, this indicates a consequence of inflammation of the dental pulp, which most often requires root canal treatment of the tooth, that is, removal of the dental pulp.

I also keep the denture in my mouth overnight, but I have a problem: it makes my mouth very dry, and my tongue sticks to the denture, which is very bothersome. My dentist assured me that he had used the best material, but I am not convinced because I did not have these kinds of problems before getting the cast metal denture. Please advise me what I can do.

Dear Sir/Madam!
The problem you describe is usually not related to the material (at least not to the metal used in this type of denture). Various forms of allergic reaction to the acrylic (plastic) part of the denture might, during a certain period, possibly present as a sensation of dry mouth. Most likely there is an actual reduction in saliva, which you noticed at the same time as the denture was changed, since a denture is in any case a certain disturbance (even if it is new and otherwise ideal). The problem can be objectified, that is, the amount of saliva secreted can be measured. Only after the actual situation has been established can we speak of a lack of saliva. Salivary deficiency can be a disease in itself or part of a broader problem, which in later stages is certainly no longer harmless. I suggest a salivary flow test.

Dear Sir/Madam! It is certainly possible to place implants. Unlike, for example, mobile canines, they do not represent foci of infection. If you are taking anticoagulant medication, preoperative preparation is required, but this does not in itself pose a risk. Have a nice day!

Dear Sir/Madam!
In this particular case, the question is how many implants are needed. In half of cases the situation is treated with one implant supporting two dental crowns. We place the implant into the bone and suture the wound. The sutures are removed after ten days. In the upper jaw, three to four months are required for successful osseointegration. After this period the implants can be loaded (artificial crowns are made), for which we need three visits over a period of fourteen days. Price (one implant):
-750 Eur (implant) X 1 = 750 Eur
-420 Eur (crown on implant) X 1 = 420 Eur
-210 Eur (cantilever unit) X 1 = "10 Eur.
Price in the case of two implants:
-750 Eur (implant) X 2 = 1500 Eur
-420 Eur (crown on implant)) X 2 = 840 Eur.
The price includes all materials and procedures from the beginning to the final result, including temporary restorations.

Generally speaking, the aesthetic defect can also be covered with artificial material if the procedure described does not succeed. Mucograft alone, without a bony foundation beneath it, usually does not have a good prognosis. For more precise information, I would need to examine the condition and perhaps take an X-ray.
