I am 56 years old and, because of periodontal disease, I am already almost without teeth. My previous dentist made both partial dentures for me, but for almost two years they have been causing me considerable problems. I wear the upper one all the time, but it makes me gag; I cannot wear the lower one at all because the tooth to which it is attached is mobile and hurts terribly when I put the denture in my mouth. To make matters worse, I crushed the upper denture when it fell out of my pocket and I closed the car door on it.
What do you advise? How should I have my unattractive teeth restored, how much does a new denture cost, can part of it be paid through health insurance and what would the remaining amount be? Thank you very much for your answer. Oh, and one more thing: how long would I have to wait to book an examination and begin dealing with my oral catastrophe?

Dear Sir/Madam!
The problem you have described is a constant occurrence in all dental practices. I assume the denture is well made and that the real problem is poor anatomical conditions in the upper and lower jaw (extensive bone resorption has resulted in insufficient height of the bony ridge on which the dentures could successfully remain stable in the mouth). Our proposal is as follows: -an upper denture without palatal coverage, thereby reducing the gag reflex. To make it we would need at least two implants (preferably four) and a new denture. Price: 750 X 2 = EUR 1500 + denture (insurance) = EUR 1500. -lower denture: removal of the painful tooth and fabrication of a complete denture on two implants. Price: 750 x 2 + denture (insurance) = EUR 1500. We offer a five-year guarantee on implants (if you do not smoke). The dentures will be stable and the upper one will no longer trigger the gag reflex because it will not cover the hard palate.

Dear Sir/Madam,
All teeth in my lower jaw are covered with crowns, but after approximately two years four crowns are already damaged: two have partially chipped and two have cracked. The crowns are all-ceramic and the teeth are subjected to normal loads. I would like to know your opinion whether this is normal. How long should all-ceramic crowns usually last? What is the usual warranty period?
Which crowns last longer: metal-ceramic or all-ceramic?
Thank you very much for your answers.

Dear Sir/Madam!
Unfortunately, I cannot give you an exact answer to any of the questions, only my opinion. In our practice we are required to give warranties on prosthetic work (7 years for fixed prosthetic restorations if they are connected). If the conditions for such warranties are impossible (we explain the problem to the patient, but the patient nevertheless insists on the treatment), we of course agree on shorter warranty periods. Therefore, the durability of crowns does not always depend only on their fabrication, but also on the conditions in the mouth, the way they are maintained, the presence or absence of the remaining dentition, ... Metal-ceramic technology is certainly more extensively proven, but theoretically the durability of all-ceramic restorations should not be lower than that of metal-ceramic technology.

I would like dental implants or stable teeth, which would make my life easier, as I am tired of constantly worrying about hiding this deficiency. Because of the lengthy procedures, waiting and the additionally high price, I have not yet been able to decide on such treatment, so I ask you to advise me how long it would take and, above all, at what price my wish for stable teeth could be realised. I have no other problems with my gums, apart from the fact that it is dreadful to wear the denture in my mouth and to worry that it will remain firmly fixed.
I would be very grateful for your answer.
Kind regards,

Dear Sir/Madam!
Solutions that provide a stable and functional fixed or removable prosthetic restoration in the upper jaw are certainly supported by dental implants: -two implants + denture with a partially reduced palate (the existing one can be modified); price: 2 × 750 = EUR 1,500. -four implants + denture with a cast base (without a palatal plate); price: 4 × 750 = EUR 3,000 + denture 710 = EUR 3,710. Optimal version -fixed circular construction that remains permanently in the mouth (we recommend the second option more); price 6 × 750 + metal-ceramic crowns 6 × 420 + 2 × 210 = 4,500 + 2,520 + 420 = EUR 7,440. We recommend the second version for EUR 3,710, in which cleaning is the simplest and most effective, any repairs are quick and simple, stability is sufficient and aesthetics are optimal. The ratio between price and result is optimal. For an exact plan and an offer, please visit our practice, where we will also take an X-ray if you do not already have one.

Hello,
It has been quite some time since I visited my personal dentist because a slightly painful lump appeared on the right side of my gum, which I... and fluid drained from it. The dentist wanted to extract the tooth, so I went to another one, who treated it and then closed it. I was glad the tooth remained, but I still felt that something was not right. In fact, the same thing kept recurring, and by chance I came to a new personal dentist who saw on the X-ray that I have 2 granulomas. I have an appointment in 3 months, and now I am worried that this will be too long and that it may no longer be possible to repair the teeth. Thank you for your answer and happy holidays.

Dear Sir/Madam!
The clinical signs you describe (a small swelling from which fluid drains, slight pain...) have in fact been indicating throughout the presence of a "granuloma", i.e. chronic periapical inflammation or a process that represents a constant focus. The decision on the type of treatment (extraction or root canal treatment) is the responsibility of the dentist, who will decide based on the clinical signs, X-ray, experience... Without a clinical examination and X-ray I cannot answer when and how the tooth should best be treated, or whether it can be treated at all. Root canal treatment of the described problem is very unpredictable, so the prognosis is poor and difficult to predict.

Some time ago two of my teeth broke and I am in quite severe pain. In Ljubljana it is extremely difficult to find a personal dentist and the waiting lists are also very long. Dear Sir/Madam!
First of all, we apologise if we missed your application. We can register you in our practice, although there is a certain waiting period. Registration would be possible in the autumn months. Since you are in pain, you can come to our emergency clinic before that date (Wednesday afternoon), but I recommend that you call ahead (you can also call the same day). In the emergency clinic we will resolve the acute situation, after which we will schedule you as described above.

I have a problem with a front tooth (upper central incisor). It is non-vital and therefore darker than the other teeth. A year ago I had the tooth whitened, but over time it became darker again. My dentist told me that the tooth is very fragile, essentially only a shell remains. I would like to know what could be done, perhaps a porcelain crown or a porcelain veneer, and of course what the approximate price of such a procedure would be.
Thank you for your answer.
Kind regards.

Now I would finally like to resolve the situation properly. I am mainly interested in the prices of such treatment and how much the first examination with you costs, so that you could assess the current situation.
Thank you and kind regards!

Dear Sir/Madam!
Solutions for replacing a lost central incisor are certainly fixed prosthetic solutions. The classic solution is to make a bridge, which means preparing the adjacent teeth into cone shapes that serve as abutments for three new artificial dental crowns (two abutments and one missing tooth). The procedure is quite radical, but it provides a satisfactory aesthetic and functional prosthetic replacement. Price: 3 × 210 = EUR 630 (metal-ceramic crown) or 3 × 350 = EUR 1,050 (all-ceramic crown). More modern approaches advise against preparing the adjacent teeth, which is completely logical from the standpoint of the health of dental and periodontal tissues. The tooth is replaced with one implant supporting one artificial dental crown. The replacement is aesthetically and functionally excellent and, most importantly, we do not affect the adjacent teeth. Price: 1 × 900 (implant) + 420 (crown) = EUR 1,320. If you decide to have the work done with us, the examination is free of charge (including X-ray).

Dear Sir/Madam! One missing tooth can be replaced in two ways: -Making a bridge: we prepare both neighbouring teeth and make three new crowns (two abutment teeth and the missing tooth). Price: 3 x 210 (metal-ceramic) = 630 Eur or 3 x 350 = 1050 Eur. -Placement of a dental implant

Price: dental implant (750 Eur) + crown on implant (420 Eur) = 1170 Eur.
