I would like 1-2 implants (I am thinking 4 and 6, with one additional pontic in between....). I will not have my panoramic X-ray until 13 July 2011. I am 66 years old, on Medrol therapy (4 mg because of TIE 6 years ago) and on anticoagulant therapy — without complications. In addition I take Fosavance tablets because of osteopenia, which is stable.
Please give me your opinion as to whether I am a candidate for an implant.
Thank you and kind regards!

Hello,
I have a 7-year-old daughter and her baby teeth are slowly being replaced by permanent teeth. In the upper jaw she already has nice central incisors; the lateral incisors have fallen out, one more than half a year ago and the other recently. That is not a problem, they will come through eventually. The problem is her swollen gums. Ever since her permanent teeth started coming in, her gums have been very red and very tense. Her dentist says this is normal and that we should massage them with a toothbrush, but there has been no change. I am worried that this will not simply disappear on its own. I kindly ask for your opinion as well.
Thank you for your answer and have a sunny day.

Tooth eruption is usually associated with changes in the gums, which may also be accompanied by broader symptoms. Brushing the areas in question is certainly welcome, as the action increases blood circulation and accelerates changes in the tissue... The changes should disappear after eruption. Without an examination it is difficult for me to assess the situation more precisely, but I am convinced that your dentist has been able to do so, so there is no need to be afraid.

Dear Sir/Madam,
I have a question regarding a procedure on a treated tooth (35). In my case, an X-ray after filling showed that the apical part of the root was not filled completely (a small, narrow part of the root canal).
I was offered the possibility of removing the root tip.
I would like to know whether checking the filling of the remaining root (and correcting it if necessary) is standard practice during this procedure among your colleagues as well, or whether this is something done only at your clinic.
After the filling had already been carried out (with an approach that was not of the best quality), I changed dentists for precisely this reason. The new dentist is now afraid to interfere with the filling again because of the risk of losing the tooth. For this reason he suggested this procedure.
I would also like to know how soon after the final (incomplete) filling the procedure should be carried out, and whether the cut part of the root is then sealed, or whether the tooth is less stable and more prone to loss because the root is shorter.
And finally, how long does the procedure take, can it be performed at your clinic with a referral, and what is the waiting time?
Thank you sincerely for your answers and help, and kind regards.

Dear Sir/Madam!
All dentists perform X-ray checks of root fillings; if not immediately, then after a certain period (we aim to minimize exposure to radiation). We perform an apicoectomy when conventional root canal treatment is not feasible (a curved canal, obstructions, apical ramifications...). If the problem is only underfilling of the canal without the causes mentioned above, I recommend repeat root canal treatment because the statistical reliability of this procedure is better. Root length is one of the main factors affecting the choice of treatment. The ratio between the length of the clinical crown and the length of the remaining root should remain 1:2. At our clinic the procedure costs 90 Eur. The procedure takes approx. 20 min.

Hello!
My upper left central incisor broke. It had been treated many years ago and had a large filling. Otherwise I had no problems with the tooth, but it still broke in half. My dentist is now treating the canals of this tooth. In ten days she will take an impression for a new tooth. The right central incisor has a similar prognosis. She therefore advised me to have the right tooth done as well. She says the two will be connected and therefore stronger. She suggests putting gold around the post in the right central incisor, saying this would make it stronger and would hold both teeth better, since they would also be connected to each other. I would like your opinion on whether gold really is that much stronger and whether it makes sense to choose gold, since the cost is then quite a bit higher. Would an ordinary post be all right? I am undecided.
Thank you for your answer and kind regards.

Dear Sir/Madam!
By "post" you probably meant a root post or radicular anchor. In my opinion, based on past clinical experience, gold root posts are certainly better than classic stellite ones (dark metal). Strength itself does not actually play the main role, since all metals used are extremely strong (resistance to fracture of the body). Unlike classic "metal" posts, gold posts do not darken the root and surrounding tissues. Gold has a high redox potential, which means that it does not dissolve in an electrolyte (saliva and body fluids) compared with other less noble metals. Significantly less decay has been observed around gold root posts, which is attributed to certain properties of the tooth that there is no point going into at this moment. If possible, I would choose a gold post.

Because the crown is completely destroyed, with only up to 2 mm thickness remaining all around and otherwise a large cavity, I decided on a new crown. I would like to know the approximate price of a crown. Which material makes the most durable crown, or which material is best for a crown?
Which material is the worst?

Dear Sir/Madam!
The type of therapy depends on the difference in size and on subjective aesthetic expectations. The crown shape can be changed with a composite build-up (a non-invasive treatment), which in some cases is not satisfactory (color, durability). If the build-up is not satisfactory, we make an aesthetic veneer or a dental crown on the tooth. These procedures are much more radical and irreversible, because prepared enamel does not grow back.

Because I have an incredible, panicky fear of dentists, I have had an upper complete denture for several years which absolutely needs replacing (it is cracked, so speaking is already very difficult, and it is very bothersome when eating and talking), and I have begun avoiding people and social situations because of it.
How much would an upper complete denture cost me, partly self-paid and partly covered by insurance, if I wanted it to be aesthetic, high-quality and to look like my own teeth (is that possible)? How much time is needed from the examination to the new denture?
Kind regards!

Dear Mr Rotar,
please answer a few of my questions. I have read most of your answers to patients' questions and gained an insight into procedures and treatment prices.
Two weeks ago my father had a workplace accident in which he fell on his face and injured his teeth. After the accident the teeth were badly displaced, but he immediately managed to reposition them somehow with his tongue (!). The dentists told him that most teeth were fine, except for the upper left central incisor, which looked fractured on the X-ray, although the root was supposedly still fine. The doctors told him that the tooth needed to be removed and an implant placed.
What would you recommend in this case (if the root and neighboring teeth are healthy)? What if the root is fractured?
If it turns out that the neighboring teeth are also damaged in some way (which is not apparent at the moment, but still), what would you recommend then?
Please also provide the prices. I know that the price of the implant and crown is 1170 €.
My father has already had quite a few dental visits (3-4), and each time he had dental X-rays taken. I am wondering whether such exposure of the head to radiation can harm his health?
I should mention that my father lives in Bosnia and Herzegovina and that the dentists there told him all of this. Since the entire procedure there has to be paid for privately and health insurance covers none of the costs, he is considering coming to Slovenia for the treatment. Please explain how many visits would be required.
Thank you very much for your answers!
I wish you continued success in your work!
Kind regards,
Jelena

Dear Sir/Madam!
In the case of a longitudinal root fracture, the tooth would need to be removed. If the tooth is fractured in the cervical area or immediately next to the root tip, it may be possible to retain the tooth with appropriate preparatory procedures (root canal treatment or apicoectomy); approx.: 100 Eur. If only the dental crown is damaged, it can be restored conservatively (build-up); approx.: 65 Eur, or, in the case of a more extensive fracture, prosthetically (crown); 210 Eur. A lost tooth can be replaced with a bridge (grinding of neighboring teeth); 630 Eur, or with a dental implant (the neighboring teeth do not need to be ground); 1170Eur.
