With a complete denture we actually have the greatest freedom in shaping the visible sector. The colour, shape and positioning of the teeth are determined together with the patient during the wax try-in stage. We often use photographs supplied by patients as a basis for aesthetic rehabilitation. The price of a complete denture is EUR 500.
Price of an implant-supported denture:

implant (EUR 900) × 4 = EUR 3600
denture with cast base × 1 = EUR 710
total = EUR 4310.

Hypersensitivity after a filling is a very common occurrence and can remain in the affected tooth for several weeks. As long as there is sensitivity to temperature stimuli and changes in osmotic pressure (sour, sweet...) and the pain disappears a few seconds after the stimulus stops, the pathological condition in the dental pulp is still reversible and in this case we wait for improvement or worsening of the condition. If the pain remains for several minutes or longer after the stimulus stops, or is even spontaneous, then we assume complete inflammation of the dental pulp. In this case the tooth also becomes painful on biting and percussion. The condition becomes irreversible and requires root canal treatment of the tooth as soon as possible (removal of the dental pulp or devitalisation).

If the first premolar has a long root and healthy periodontal tissues and is therefore stable, we could consider making a similar construction to the one you have had until now, with the difference that the first molar would be loaded only with a small rest (which does not require preparation of the tooth) to additionally transmit chewing forces. However, you should in any case trust the decisions of your dentist.

Considering your age and dental status, I would advise against treatment with a removable denture. As the first treatment option, I would certainly suggest dental implants. Insufficient bone volume is only a relative contraindication, since lost bone can be augmented and sufficient primary stability of the implant can thereby be ensured. A decision on this could be made after examining an orthopantomogram and performing a clinical examination. The second treatment option is a conventional bridge construction, but compared with the first it is somewhat more radical because the supporting teeth must be ground down; however, it is incomparably more effective and suitable for replacing teeth than removable prosthetics.
A partial denture (acrylic) is fully covered by ZZZS when provided by an authorised dentist.

The concept of stability or instability with removable prosthetic restorations is very relative. Patients sometimes complain about dentures that, in our opinion, have completely satisfactory stability, while on the other hand they are sometimes satisfied with prosthetic restorations that do not meet accepted standards in any respect.
In any case, it is difficult to expect the denture to remain completely motionless during function, because some instability is already due to the mobility of the mucosa (that is, the supporting surface itself). The number of retention elements (remaining teeth) is an important factor affecting denture mobility. In your case there are only two support points, so the expected stability probably cannot be achieved by this method alone. For any further information, a clinical examination would be necessary. An edentulous upper jaw can be restored with a denture that leaves the palate uncovered only if the denture is supported (retained) by dental implants. We usually need four implants for retention; they carry ball attachments or are connected with a bar. Matching components in the denture attach to the retentive parts of the implants. The denture can still be removed, which allows good hygiene of the denture itself, and especially of the retentive components and the mucosa (gums) around them. Patient satisfaction with this type of denture is incomparable with conventional treatment.

Aesthetic restoration of the intercanine sector can be carried out in several ways:
-orthodontic treatment (fixed braces), which provides the additional missing space for the normal width of the lateral incisors, followed by placement of two implants at the sites of the lateral incisors. The treatment is lengthy (at least one year of wearing braces, implant placement (half a year for osseointegration), and only then fabrication of crowns on the implants.
-fixed prosthetic treatment (grinding the canines and making two new crowns on each side). The treatment is fast with a good and durable result, but it is radical because grinding intact canines is a procedure at the limit of accepted doctrine.
-fabrication of adhesive bridges to replace the missing lateral incisors (an adhesive bridge is only bonded to neighbouring teeth and does not require grinding of the adjacent tooth). The treatment is quick with a good aesthetic result, but compared with the previous two options the restoration is considerably less reliable, because under overload the adhesive can fail and the replacement tooth can come off.
