Dear Sir/Madam,
in March I received my first night guard because I am a very severe bruxer—I do not grind my teeth, but I clench very hard.
The guard is the softer version, 4 mm thick rubber, worn on the upper teeth. After a month and a half I noticed that after using it my tongue, gums and palate burn abnormally—only the areas that are in contact with the guard. (At night my tongue rubs against or touches the guard.) The rubber also became softer, as if this plastic were slowly dissolving in my mouth. I received a new one; it was fine again for 3 weeks, then after a certain period of use the burning sensation returned. My tongue is also very red, and moving it even hurts. Even if the condition settles during the day, as soon as I put the guard in my mouth it literally starts burning. I often can barely fall asleep. I trimmed the guard only slightly in two places because it pressed near the lateral incisors, so that I could wear it.
I am VERY worried about what is happening. Could some toxic substances actually be released into my body? Especially because the plastic becomes thinner with use—the material must be going somewhere, right? The burning is unbearable, but without it I have other problems. In short, a vicious circle. In fact, for the last 2 months I have also been feeling worse, with constant nausea, dizziness.... I am worried that I am poisoning myself. I am also planning a pregnancy and I am terrified that anything might pass to the fetus.
Where can I find out the exact composition of these guards? What substances are used? Is this softer one available only in plastic or are there other materials? I already find it disturbing that people put these artificial materials in their mouths at all... seeing the consequences in myself, I am of course frightened.
Thank you for your answer.

The dental technician who made the guard can prepare a powder sample of the material for use in testing. If the test is positive, a guard can be made from another material (silicone).

Dear Sir/Madam,
For some time I have had problems with my upper central incisor. A year and a half ago I had veneers made because of diastemas, and they were made very nicely. Shortly afterward the central incisor started hurting. Nothing was visible on the X-ray, the tooth was vital, so the doctor said we would wait a little and perhaps the pain would settle.
Since this did not happen for several months, he opened the tooth, devitalized the nerve and treated it. Then he closed and filled the tooth. The pain did not stop, so he treated it again.
Even after that the pain is still present. It does not hurt spontaneously as much as before, but if I touch the tooth, the pain is still very severe. I also have pain under the nose on pressure.
The dentist says the pain will subside and that I should be patient for a while. Nothing is visible on the X-ray, the filling looks very good, yet the pain is still present. It has already been two months since the last filling.
What could this be and, in your opinion, is there any other solution?
Thank you very much in advance for your answer.

I suggest another X-ray (a few months after the last filling). Only after some time do changes in the periodontal tissue at the root apex actually become visible. The X-ray will either confirm or rule out suspected periapical inflammation, which is presumed to be the cause of the pain you feel, although sometimes we simply cannot confirm the symptoms reported by patients with diagnostic procedures. Ultimately, if the tooth hurts and the X-ray shows a good root filling, the next step in treatment is apicoectomy (root-end resection).

Removing custom-made posts can be quite a risky procedure because the root may fracture. After twenty years, however, it most often happens that the posts simply come out of their seats, in which case replacement is logical. Even if a metal post cannot be removed, its dark show-through through an all-ceramic crown can be blocked with opaque coatings so that the optical disturbance disappears. Price of three zirconia crowns:
-350 Eur X 3 = 1050 Eur.

Dear Sir/Madam,
My dentist found that I have a fractured root under the crown on the upper lateral incisor. Since the entire bridge depends on this root, I have been postponing for several years having the root extracted as my dentist suggested. Inflammation collects around the root; I do not have major problems, occasionally I feel it when it is cold, warm, etc... He also told me that there is no other option except a denture. I cannot come to terms with that, as I am only 50 years old. The bridge I currently have is still holding for now, but I have had it for more than 10 years and something should be done because it is starting to lift a little.
Please, is there any other option regarding the fractured root? Could it somehow be saved?
Do you advise me to wait until everything collapses by itself?
Most importantly, how much does this inflammation affect the whole body? Could it become more serious or dangerous over time?
Thank you for your answer, please understand me,
best regards
Klavdija

I can suggest a solution if I can examine a panoramic X-ray. Persistent inflammation is certainly not appropriate, as it can cause local problems (bone resorption, ...) and possible systemic disturbances (swollen lymph nodes, elevated body temperature...). If the root is fractured vertically or horizontally in the middle third, it needs to be removed.

Dear Sir/Madam!
I have complete removable dentures in both the lower and upper jaw.
I last replaced them five years ago, but the teeth are worn, too short and no longer sharp.
Until now I have always had this done at a public health center, at the expense of the insurance company.
Now the waiting time is too long, especially because I live in Africa and will only stay here until September 20.
Please tell me whether it is possible to make new dentures between Aug. 15 and Sept. 15, and I would also like to know the price.
I want completely ordinary dentures, the kind you receive through the health insurance clinic.
Best regards, Barbara

It is the upper lateral incisor, which is quite visible. I whiten the other teeth with whitening strips and the difference is becoming too obvious.

Dear Sir/Madam!
Considering that the tooth in question is rather slender and has been additionally weakened by devitalization and removal of dentine during root canal treatment, I suggest making an artificial crown on a root post, which can be metal or made of other materials (glass fiber, zirconia...). This provides complete aesthetic and functional restoration of the tooth (reducing the likelihood of fracture). Bleaching would only bring us closer to an aesthetic correction, but would certainly not strengthen the tooth. Price of the proposed procedure:
-350 Eur (all-ceramic crown)
-30 Eur (post)

It was held most tightly by the clasp on tooth 1 right. On Thursday I went to the dentist, who pulled the denture out, which hurt very, very much. She loosened the denture and said that
I would now be able to remove it much more easily and would get used to it. However, I still cannot do it and I am afraid of the pain.
Today the dentist went on vacation and will return on July 4.
Please tell me whether I can keep the denture in my mouth for 14 days until the dentist returns and clean it there, or whether I absolutely have to
remove it from my mouth.
