Hello,
Just under a month ago I had endodontic treatment of my upper first premolar. The endodontist treated the canals using an operating microscope and rotary instruments. After the first treatment session, he placed medication in the tooth, and for 3 days I had very severe pain. At the second visit he enlarged the canals further, cleaned them with ultrasound, and completed the root filling. He filled them quite deeply. For about 14 days I felt pain in the tissues, although it was not unbearable. It then settled, but a week later I started feeling pain again (it appeared after I clenched my teeth while sleeping). I also feel pain when the tooth is tapped. The temporary filling is not too high and does not bother me. I would like to know whether this course of pain can be normal, for example as a reaction to the filling material, because one root was very long and the filling was done quite deeply. How long can pain in the treated tooth last?

Dear Sir/Madam!
The root filling should be made to the end of the root, or more precisely to the point where the root canal narrows before exiting the root (physiological foramen). The filling can be checked on an X-ray, because its endpoint and homogeneity can be seen relatively well. Pain after root canal treatment is a relatively common problem and depends on various factors (despite a good X-ray image). It is not possible to say how long it may last. There are cases in which, despite an excellent X-ray appearance, additional therapeutic procedures were necessary (repeat root filling, apicoectomy...).

Dear Dr Rotar,
a piece of the crown of my upper first molar chipped off on the outer side. It does not hurt, the filling is still in place, and I pressed the broken piece (two cusps joined together, broken halfway toward the gum margin) back into its original position. Can the piece of tooth be bonded back? Or does that make sense? These are permanent adult teeth. What else could be done? It happened on Sunday afternoon :(
What actually worries me more is what is happening to my teeth. About two years ago the pulp of one tooth died (caries under a filling). Less than a year ago, caries developed under a filling in another tooth; we tried partial pulp removal, and for now it is fine. Now this piece has broken off. On some teeth I have a veneer-like layer made with flowable porcelain near the margin for aesthetic reasons, because demineralisation occurred there (like a tiny channel along the gum line).
I have already asked my dentist whether perhaps I should replace the fillings because they may be too old. (All my fillings are white, and the oldest is about 10 years old.) I did not get a definite answer because the panoramic X-ray looked completely fine. I would rather go through replacing the fillings than have things like this keep happening. Would it be a good idea to consider replacing the fillings?

In the case described, it does not make sense to bond the broken piece of tooth back into its original position. Appropriate treatment would be a partial or full crown. We replace fillings when there is a sufficient reason to do so: poor marginal seal, fracture of the filling, or discoloration of the material in the visible area. It is difficult to state the reason for the other problems you describe without a clinical examination and an X-ray.

Hello!
I have a question. A small part of a tooth root is visible at my upper gum, and I would like to know how or with what it could be covered, if that is possible at all. It is true that I use snus (fugo), and I am already a little worried about this tooth. I would really like to know whether there is any cream, solution, or anything else that would actually be effective, for example to accelerate gum growth or healing around the tooth root. I am already using Gengigel, but there is no visible improvement. I would really appreciate your answer as soon as possible, and I sincerely thank you for it!!

Dear Sir/Madam!
The gum around the tooth root is inflamed because of chronic irritation from sharp edges and plaque accumulating at that site. The root needs to be removed or, if it is still possible, restored with a crown and post. The best and only measure against gingival inflammation (once the work in the dental office has been completed) is mechanical plaque removal, i.e. brushing.

I would like to know how much it costs to replace one tooth. I am missing a first molar. Do two additional teeth have to be ground down, or can an artificial root be made? I am interested in both options.

A missing first molar can be replaced in two ways: -a conventional bridge can be made, which requires grinding the two adjacent teeth into the shape of truncated cones and making three crowns: two abutment crowns and one pontic. Price: 210 Eur (metal-ceramic crown) X 3 = 630 Eur. -placing an implant (dental implant) is a better and more modern solution because it does not require grinding the adjacent teeth. The implant replaces the lost root and supports an artificial dental crown. Price: 750 Eur (implant) + 420 Eur (crown on implant) = 1170 Eur.

It has become so bad that I cannot chew food at the back because it hurts so much. My other and biggest problem is that I am extremely afraid of the dentist; I almost faint every time I sit in the chair. I simply have a phobia. I would like to know whether this could be done so that I would not feel anything. I know it is urgent, but I cannot overcome the fear. Please answer me. Kind regards, desperate.
