Hello,
because my tooth hurt, my dentist replaced the old silver filling in my upper first molar with a new white filling. The dentist told me there was nothing wrong with the tooth and that the pain was only a consequence of the flu, because it hurt only when I moved my head or ran, otherwise not, and nothing problematic was found inside the tooth either. A week after the filling was replaced, however, the tooth became very sensitive and painful on biting, and now it has also become sensitive to sweet and cold; it gives a sharp pain even when I smile in cold air. The dentist told me this was only a reaction to the white filling and that the problems should pass within a few weeks. It has now been well over a month and there is no improvement. In addition, I often have a taste of pus in my mouth, not on the same side as the painful tooth but lower down. I would like to ask for your advice on what this could be, since I have never had any problems with my teeth before. Thank you and kind regards!

Dear Madam!
Pain after replacing a filling is caused by irritation of the dental pulp, which occurs due to vibration, temperature changes and drying of the dentin during drilling. These factors cannot be avoided. The depth to which the carious process has to be removed is also essential; the closer it is to the pulp, the greater the likelihood of excessive irritation. Irritation of the pulp causes inflammation, which you experience as unpleasant sensations: hypersensitivity to temperature changes, pain... If the inflammation was partial (hyperemia), the problem is reversible and does not require additional treatment; however, if the inflammation is more severe and involves the entire pulp (pain on biting, percussion), the condition is irreversible and requires additional treatment: root canal treatment. Darkening of an incisor in the way you described is characteristic of non-vital (dead) teeth. Medications and other causes discolour teeth somewhat differently. The dark colour of dentine showing through at the neck of the tooth, where the enamel is thinner, raises suspicion of the diagnosis described above. The causes of tooth death can be very different, but most often the cause is trauma to the tooth, which may have occurred a long time ago. Trauma also includes minor blows that you may not even have noticed at the time. Preparing a tooth for a filling or deep caries are also common causes of this condition, but since you did not mention these in your question, I assume that the darkening is due to the cause described above.

Yesterday I had 2 teeth, 6 and 7, extracted and the wound was stitched. I would like to know how long bleeding normally lasts. I take Marivarin; I have now stopped it and am receiving Fragmin 7500 E injections. Is it possible that the oral surgeon left something next to the tooth as a kind of drainage so that the blood can drain out, so that it does not become infected?
Thank you in advance for your answer.
Kind regards

Suturing the wound and placing collagen preparations in the wound certainly help to stop bleeding and promote wound healing. Cooling is also recommended. As regards taking Marivarin and Fragmin, follow the instructions of the specialist who prescribed them.

I have tried mouthwashes and cleaning my teeth more frequently, but it did not work in the long term. I must admit that I have not been to a dentist for a long time and my teeth are somewhat irregularly positioned. I would like to know what the treatment for this is and, ultimately, the price. I would be very grateful for your answer.

But then the surgeon told me that my dentist should take care of the further prosthetic restoration, for which he is not qualified. Since I have prosthetic treatment approved by ZZZS, I would like to know whether you could place the abutment and prepare everything for prosthetics, which my prosthodontist would then complete. Dear Madam!
Since whitening is a purely aesthetic service and has no essential connection with dental health, it is of course logical that the teeth should be treated and restored before the procedure; in other words, services that are more urgent must be carried out before aesthetic services. If the teeth have fillings, after whitening it is usually necessary to adjust the colour of the fillings, at least in the intercanine region (front teeth). I hope I have answered your question. An appointment must be made in advance for whitening.

Whitening the teeth in the lower jaw is possible because you do not wear braces there.

Dear Sir/Madam,
I regularly visit my dentist, but I have major problems with my teeth and I am thinking about how to proceed.
Some time ago I neglected my oral hygiene a little and developed some caries near the gums. My dentist treated the teeth, removed the damaged enamel in a U-shape along the gum line and restored the areas with white fillings.
The problem is that despite better hygiene, the teeth are still decaying at the gum line (after one year). I have also stopped taking medicines that were supposedly causing bad teeth, added Ca and Mg, and use an anti-erosion toothpaste, but the condition keeps getting worse.
I am somewhat worried about all this. I am 28 years old and would like to ask for your advice. Is there any protection against this problem (I brush twice a day, eat 5–6 small meals, follow a special diet with fewer carbohydrates — which can supposedly cause acidification of the body)? Perhaps some deep cleaning by a dentist (price?), coatings or restorations (porcelain, veneers) — because if this continues, I will soon lose my teeth. I would be very grateful for your advice (my dentist only recommends brushing, but that does not seem to be enough?!).
So I would like to know what solution you recommend considering my age and the condition of my teeth (I would like to keep my own teeth) — and also the price, since I am still a student and dependent on my income. I would also like to know whether any of these services are co-financed by insurance (above-standard treatment). I personally tend toward porcelain restorations, but I am not an expert, and my dentist does not want to hear about anything other than white fillings... If you recommend one of the procedures, would you recommend it for all teeth or only the 'visible' ones, and is it better to perform the procedures gradually — a few each year — or all at once, and what is the service life of the individual 'improvements'?
I look forward to your answer.
Kind regards,
Urška

Dear Madam!
From what you have written, you apparently have less well-mineralized enamel. A poorer mineral structure of the enamel and continuously inflamed marginal gingiva, which acidifies the surrounding environment, contribute to rapid dissolution of the enamel (caries) at the tooth neck, which is also what you describe. Gingival inflammation (gingivitis) is eliminated with proper and regular oral hygiene, since its main and most often only cause is the presence of bacterial plaque. The dentist should monitor two parameters that are the main indicators of the cause of gingivitis: the plaque index and the sulcus bleeding index. The solution to the problem is sought in its causes: cleaning (the plaque index and bleeding index should both be 0) and improving enamel quality (topical fluoridation). If the lesion is already present, the preventive measures listed are of course insufficient. The lesion must be removed and a filling made. I am personally not enthusiastic about white composite materials, since in the long term they do not perform best. The fabrication of a crown or another prosthodontic restoration depends on the proportion of destroyed dental tissue. We could discuss this after a thorough clinical examination.
