I assume that you have swelling next to a tooth, caused by a purulent inflammation in the bone around the root tip of the affected tooth. To assess the type of treatment, we would need to examine X-rays, which you can also send us by e-mail if they were taken digitally. If we assume that the tooth can be treated with conventional root canal therapy, we can estimate the treatment cost at EUR 200.

Dear Sir/Madam!
Teeth grinding (bruxism) is a disorder that we encounter increasingly often in practice, which is also thought to be a consequence of the everyday stresses patients experience. The causes can also be simpler: iatrogenic (caused by our treatment) or due to changes in the mouth that have arisen in some other way and cause certain changes in jaw relationships. Bruxism is treated with splints (a custom-made protection for the teeth that you wear at night). The splint protects the tooth surfaces during grinding and causes the loss of "muscle memory", which ultimately leads to a change (relief) of the entire stomatognathic system. The splint is made quickly and easily, so do not delay having one made.

Dear Sir/Madam,
a few days ago at a party someone accidentally hit me in the mouth with an elbow, so my gum split above the upper incisors and the tooth became very loose. I had swelling and a swollen gum. The right incisor hurt so much that I did not eat for 3 days, and now it has begun to be very sensitive, especially when I drink colder water. The whole tooth hurts with any change in temperature. The tooth itself is not damaged and looks exactly as it did before. Is it possible that the pain comes from the injured gum if the wound is deep enough? Five days have already passed, but the pain remains. Please give me advice as soon as possible.
Thank you

Dear Sir/Madam!
The injury you described is not insignificant, so you will most likely need a dentist's help. Tooth mobility after an injury suggests several possible diagnoses: -fracture of the tooth root in the middle or cervical third -tooth subluxation and severe injury to the periodontal ligament, the membrane that anchors the root in the bone. -fracture of the alveolar bone together with injury to the periodontal ligament. In all of these cases, necrosis (death) of the dental pulp will probably occur, followed later by gangrene when bacteria colonise the dead tissue. The problem requires root canal treatment (removal of the dental pulp) and filling of the root canal with thermoplastic materials. Injury to the periodontal ligament requires immobilisation (attaching the tooth to neighbouring teeth for a period of time so that the attachment ligaments can recover or be replaced by scar tissue; the tooth becomes firm again). A root fracture in the middle third of the tooth will most likely be fatal for the tooth. Sensitivity of the tooth a few days after the injury actually gives hope that the tooth is still vital and may remain so, but it still needs to be immobilised and root canal treatment postponed until the tooth loses vitality. In this case, we recommend periodic vitality testing of the tooth and, if the pain increases, immediate root canal treatment. An X-ray will also facilitate diagnosis. Visit a dentist.

Dear Madam!
Ceramic veneers are among the more demanding fixed prosthodontic procedures, and their indication is strictly aesthetic. Teeth with extensive fillings are usually not suitable candidates for aesthetic veneers, because veneers need a fairly firm and intact substrate for cementation. In such cases we prefer to make crowns, which can also be highly aesthetic, because in addition to aesthetic restoration they provide functionality and durability. We could discuss your case more specifically after a clinical examination and review of a panoramic X-ray. The price of a veneer is EUR 350.

Dear Madam!
I assume that you are missing two teeth. With normal anatomy of the lower jaw (sufficient bone volume and a normal course of the nerve), the best solution is placement of two implants and fabrication of two crowns on them. This avoids preparation of the neighbouring teeth. If we exclude that treatment option, a bridge is appropriate. In that case both neighbouring teeth are prepared (an invasive and irreversible procedure involving hard dental tissues), which is also not an unreasonable decision if these teeth have extensive fillings or carious lesions. If the neighbouring teeth are intact, the decision should favour implants; if the neighbouring teeth are diseased, it should favour a bridge. Price: -implant EUR 900 x 2, crown on implant with abutment EUR 420 x 2 = 2640 -bridge: EUR 210 x 4 = EUR 840.

Dear Sir/Madam,
I would like to ask for an answer to the following question. I have problems with my upper first molar. It is a root-treated tooth in very poor condition; the filling fell out several months ago, after which my dentist only placed a temporary filling, which has now fallen out again. Very little of the tooth remains, and my dentist's prognosis is that the tooth must be removed because there is also a large granuloma above it. Is that really the only solution? I also asked her about the possibility of placing an implant, but she says this will be a problem because I have a very low bite. What is your opinion — does a low bite really present a problem for implant placement?
What is your opinion of a cast partial denture?
Can I bring a panoramic X-ray that is a few months old to an examination with you?
Thank you for your answer and kind regards.

Dear Madam!
Given the problems you have described, I would definitely like to see the condition 'in person'. Under the term deep bite, I do not see problems that could affect implant placement at the site of the first molar. There may of course be other reasons that make implantation more difficult or impossible, which your dentist was trying to explain to you. A panoramic X-ray is welcome and necessary. A clinical examination and X-ray will give us all the necessary answers, so please make an appointment by phone (040294488 or 04/5300660). Please ask a more specific question about a cast partial denture, because with such a general question we could get too involved in matters that do not interest you.
